Ushahidi, which translates to “testimony” in Swahili, was developed to map reports of violence in Kenya after the post-election violence in 2008. Since then, thousands have used our crowdsourcing tools to raise their voice.
It can give data through polls as "multiple data types", it can aslo track posts through social media, and sms text responses.

Multiple maps: Map tiles including street and satellite provided by Open Street Maps, MapQuest, and more
Data sources: Map and visualize data streams from third parties like Twitter, Twillio, SMSSync, Nexmo, FrontlineSMS, and email

The app was developed in partnership with Omidyar Network, Cisco, Ford Foundation, Google.org, Humanity United, MacArthur Foundation, Rockefeller Foundation, Knight Foundation, and the USAID (from the american people).
This, like many apps, don't exactly work in rural areas in which people are unable to have access to such amenities as cellular devices.
