This policy applies to all people in America who meet the eligibility requirements and are in a state who has expanded to medicaid coverage. They must apply and meet certain criteria in terms of income, household demographic and marital status.
In terms of first responders, not much of this policy will effect them due to the fact that issues such as insurance and medicaid are not really an issue handled by EMS. The only thing I can think of is how this might effect a patient possibly worrying about insurance and wanting to refuse EMS services due to money issues. This might also play a role in where patients are taken as well, but more likely not since no matter what their insurance, etc is, they'll be transported to the hospital of their choice or depending on their condition, cardiac arrest-->closest hospital.
It was harder to find this information, but from what I gathered, the concept of national health insurance first became a topic of discussion during the Truman administration. From 1958 to 1964, controversies rose and a bill was drafted, specifically by who is unknown, but the signing of the act was part of Johnson's Great Society. Though names are not listed, it is said that those who previously worked on the King-Anderson Bill drafted this current Medicaid policy and that Wilbur Cohen, Assistant Secretary for Legislation of the Department of Health, Education and Welfare, really pushed for this Medicare bill.
This is seen as a good start to a major problem that needs far more assistance and advancement. People cite that meeting eligibility requirements are very hard and there are people that make more money than allowed to qualify, but still cannot afford mental health treatment. This policy is appreciated in a certain capacity, yet it is very apparent that people want more and think more help is needed. Of course, you will always have an opposing side when it comes to politics and there are people who think this is the perfect amount of assistance and nothing else is needed.

This policy specifically denotes what defines an institution, determining whether or not an institution is IMD and other aspect such as ages, costs and the definition of IMD are also set forth. Ultimately proving to set parameters and expectancies to those people and institutions that find themselves operating within this policy or referencing it.
